12 Apr New partnership helps navigate the NDIS
Around nine months ago, Dolie Ufi from Kildare Road Medical Centre in Blacktown, called the Uniting Local Area Coordination (LAC) program front desk at Parramatta and spoke with Jessica Bain, Community Engagement Officer, about the confusion that general practitioners and patients felt towards the National Disability Insurance Scheme (NDIS).
Local Area Coordinators assist people with disability to access the NDIS and work with our customers to identify their goals for their NDIS plan. LACs also support customers to connect with their community supports as well as exploring ways to achieve their goals in their NDIS plan.
The beginning of a successful partnership
Jessica arranged a meeting with Dolie to discuss how the medical staff and patients felt, and the barriers they faced when looking into the NDIS.
Weeks later, Kildare Medical Centre launched the first outreach program session in the form of a community connection desk. Uniting LACs were based at Kildare medical centre on a fortnightly basis to support any patients who had questions about the NDIS.
Administrative staff at the medical centre assist with booking patients in to meet with a Uniting LAC where they can have one-on-one support to understand how to access the NDIS or understand their current NDIS plan.
Looking ahead to the future
Around five patients meet with a LAC at the medical centre once a fortnight. The initiative has been such a success that patients from neighbouring GP clinics are looking to book into one-on-one support with a LAC. The LAC team also refer patients to community organisations for support.
Currently, a Uniting Local Area Coordinator is on site at Kildare Road Medical Centre fortnightly on Wednesdays from 10.00am – 12.30pm.
The next step is to branch out to other GP practices.
